You will need an Affidavit and a Motion for Permission to Appear.

How do I apply?

Register online with the Ohio Supreme Court Office of Attorney Services at http://www.supremecourt.ohio.gov/AttySvcs/PHV/. Upon completing the registration requirements, you will receive a Certificate of Pro Hac Vice Registration by e-mail. File the Motion in the court in which admission is sought. If granted, the Office of Attorney Services should be notified through the online system.
